Patient and payer

  • Patient demographics and contact information
  • Emergency contact and language preference
  • Insurance information and authorization details
  • Expected discharge date and care location

Orders and providers

  • Signed home health order
  • Diagnoses and requested disciplines
  • Ordering provider’s name, NPI, telephone, and fax
  • Provider who will follow the plan of care

Clinical records

  • Face-to-face documentation when required
  • History and physical or discharge summary
  • Medication list, allergies, and precautions
  • Wound, therapy, laboratory, or equipment information as applicable

Do not send patient information by ordinary email or through the public website. Fax referral documents to (310) 527-5558 using your organization’s approved privacy and security procedures.
