Provide skilled assessments, patient and caregiver education, care coordination, and timely clinical documentation for patients receiving care at home.
- Active California RN license
- Home health experience preferred

Home Health Licensed Vocational Nurse
Support physician-directed home health care, reinforce teaching, report changes promptly, and maintain accurate, timely clinical documentation.
- Active California LVN license

Rehabilitation · Field-based
Physical Therapist
Evaluate mobility and function, establish individualized plans, coordinate with the care team, and help patients progress safely toward meaningful home goals.
- Active California PT license
