Palliative care provides an added layer of support for people living with serious or complex illness. It focuses on reducing symptom burden, helping patients function as safely as possible at home, and improving communication among the patient, family, clinicians, and treating provider.
Who may benefit
The program may be helpful when illness causes difficult symptoms, repeated changes in condition, medication or self-management challenges, reduced strength or mobility, caregiver strain, or uncertainty about the care plan.
Treatment can continue
Palliative care may be provided while a patient continues disease-directed treatment. The patient does not have to stop treatment or elect the Medicare hospice benefit to ask whether palliative home health is appropriate.
Eligibility is individualized
Services are based on a provider’s order, clinical assessment, skilled need, homebound status when required, the plan of care, and payer-specific coverage requirements.
